Weight and doses, together
Their weight curve colour-coded by dose, with the dose timeline and adherence alongside — so a flat fortnight has context, not panic.
Your clients live with their GLP-1 every day — and track it in Traqr. The Care Team portal shows you the trends they choose to share between appointments: doses, weight, appetite, side effects. Free for practitioners today, and always their call.
A wellness companion for your practice — not a medical device, and not a patient record.
Linking takes minutes and puts your client in charge of every category they share — which is exactly why they’ll say yes.
Sign up at the portal with a verified email, two-step sign-in, and the Care Team recipient terms — it only takes a few minutes.
Issue a one-off code from the portal and pass it on however suits you both. Codes expire after 14 days if unused.
Your client enters the code in their Traqr app and switches each category on or off — and can change their mind, or stop sharing, at any time.
From their next check-in you’ll see what they’ve shared — real numbers between appointments, not recollections in the room.
Everything below appears only for the categories each person has chosen to share — and it’s the same pharmacology-aware view their app is built on.
Their weight curve colour-coded by dose, with the dose timeline and adherence alongside — so a flat fortnight has context, not panic.
Hunger, fullness and food-noise check-ins across the medication cycle — the part of the story a set of scales never tells you.
Gentle flags when something shifts between visits: a plateau, a late dose, a run of tough side effects, rapid change, low protein.
A roster of the people you support with their latest shared numbers at a glance — and only the categories each person chose.
A modelled view of where a trajectory could be heading, drawn alongside their real readings — an estimate to talk about, not a promise.
Export a per-client PDF summary or a caseload CSV — every export is marked as shared with consent and covered by the recipient terms.
The portal only ever shows what each person has switched on. That’s not a limitation; it’s why accepting your invite is an easy yes.
Seven categories, each one their choice — switched on or off at any time.
If they stop sharing, your access ends straight away. Reports you exported earlier aren’t recalled — which is why every export is marked as shared with consent and covered by the recipient terms.
Each time you open someone’s data, an audit record is written. Respecting the share isn’t a promise — it’s recorded.
Free-text notes, meal photos and contents, and community activity stay private to them, full stop. Nutrition arrives as daily totals only.
The Care Team recipient terms keep shared data confidential, never for marketing or onward use. Read how sharing works in the privacy policy.
Traqr Care Team is for individual professionals using it in their own practice, at a client’s invitation.
See how the weeks between visits are actually landing — doses, side effects and weight as they happened, not as they’re remembered in the room.
Ground each session in what really happened since the last one. Celebrate the wins that are real and skip the guesswork.
Daily nutrition and protein totals alongside appetite signals — enough to guide, while meals, photos and notes stay private to them.
It isn’t an organisational patient-management system, and it isn’t a medical record — organisations should get in touch at support@traqr.health.
What practitioners ask before their first invite — cost, privacy, and what your client controls.
The portal is currently free for the people clients share with — practitioners, coaches and dietitians. Your clients subscribe to Traqr; you just need an account with a verified email.
Your name and email — the address their data is being delivered to — plus exactly which categories they are sharing with you. They can change what they share, or stop sharing entirely, at any time from the Traqr app.
Your access ends straight away — their data disappears from your portal view. Reports you exported earlier are not recalled, which is why every export is marked as shared with consent and covered by the Care Team recipient terms.
No. Traqr is a personal wellness tracker, and the portal shows trends people choose to share. It offers no medical opinions and no dosing advice, and it is not a system of record for clinical care.
The Care Team portal is for individual professionals invited by the people they support — it is not licensed for organisational deployment, and no business-associate relationship is created. Organisations wanting Traqr in their workflow should contact support@traqr.health.
Portal accounts require a verified email and two-step sign-in. Every time you open someone’s data an audit record is written, exports are marked as shared with consent, and your use is governed by the Care Team recipient terms. Data is stored on Google Cloud in the United States (encrypted in transit and at rest), as described in the privacy policy. Free-text notes, meal photos and community activity are never shared at all.
Create your free account, send your first invite code, and see the difference real trends make to your next conversation.
